Structured Family Caregiving (SFC) is a Georgia Medicaid program that pays a family member to be the live-in caregiver for an aging or disabled loved one — instead of placing them in a nursing home.
Tax Note: The payment is classified by the IRS as a difficulty-of-care payment under Notice 2014-7 and is excluded from federal income tax for caregivers living with the care recipient.

Yes. SFC requires the caregiver to be available full-time and live in the home with the care recipient. It is a full-time caregiving role, not a part-time supplement.
For caregivers who live with the care recipient, yes. Under IRS Notice 2014-7, difficulty-of-care payments from Medicaid waiver programs are excluded from federal gross income. We recommend consulting a tax professional for your specific situation.
No. Federal Medicaid rules prohibit spouses from being paid as SFC caregivers. The same rule applies to legal guardians and parents of minor children.
You start at $550/week. After 6 months in good standing, your pay automatically increases to $600/week. After 18 months in good standing, it increases again to $650/week. These raises are written into your offer letter from day one.
